Philhealth is a leading health insurance provider administered by our government to handle the health insurance needs of every Filipino. A Filipino citizen of legal age is encouraged to be a member of the said agency considering the affordable monthly contribution for paying members to avail a health care designed for every individual.
Membership Classification
Members in the Formal Category
A category to include members who have formal employment contracts or fixed terms of employment to include both government agencies and the private sector. Their premium contribution is divided between the employer and the employee.
Members of the Informal Category
Those who are self-earning individuals or migrant workers. These includes a wide sector of individuals who pay their monthly contribution voluntarily.
Indigent Members
Persons who are identified by the Department of Social Worker who have no fixed means of livelihood or insufficient income for their daily subsistence.
Sponsored Members
Covers individuals whose payment contribution is being paid for by another person either government agency or a private individual.
Lifetime Members
Are paying individuals who had paid at least 120 monthly contributions to the agency and having reached the retirement age
Senior Citizens
Members who are 60 years old and not currently covered by any existing membership classification.
Requirements for Unemployed or Self-employed Members
How to be a Philhealth member?
Members who are not currently employed formally in any government or private entity or a self-employed individual can register as a voluntary member. Foreigners living in the Philippines can also become a member under this category. To register, you have to submit the following requirements:
- Filled out PMRF (PhilHealth Membership Registration Form) -this is available in any Philhealth office or branch or you may download it online.
- NSO Birth Certificate/ Valid ID – photocopy of your NSO Birth Certificate but you have to present the original for verification
- NSO Birth Certificate of Dependents– photocopy or as supporting documents
- Alien Certificate of Registration – photocopy of ACR issued by the Bureau of Immigration (for Foreigners)
You will be issued a lifetime Philhealth Number and directed to pay your corresponding contribution. You must not forget to update payment either monthly, quarterly or annually at your Philhealth office or accredited banks or payment center. You have to keep your receipt for future reference.
Requirements for Employed Members
For employed members, you have to present the same requirements as Individually paying members, you just have to indicate your Employed Status in your PMRF.
Philhealth OFW Requirements
All Overseas Filipino Workers can pay their monthly contribution upon registration with the Philippine Overseas Employment Authority (POEA). The requirements are:
- Filled out PMRF Form
- Supporting documents of the beneficiaries
- Proof of being an active OFW
If you are already a PhilHealth member, you may continue your payment and update your membership status as OFW.
If you are staying abroad but want to be a member of Philhealth, you need to submit the following at the Philhealth Office
- PMRF
- PPPS (PhilHealth Premium Payment Slip)
- Valid Identification Card (photocopy)
- Payment of monthly contributions to accredited Philhealth collecting agent
PhilHealth Sponsored Members and Indigents
Sponsored members by government agencies like LGU, Congress, etcetera or by private entities or Indigents as categorized by DSWD as persons belonging to 25% of the lowest income earner individuals may submit the same forms for completion.
