Under the Philippine Law, a Filipino citizen either working on a company (Employed), having a business or other source of income (Self-employed), and or persons who choose to become voluntary paying members, should register at the Social Security System for membership.
All income earners are required to contribute monthly into the fund depending on their salary bracket in exchange for the insurance benefits they may enjoy with the membership like sickness, maternity leave, retirement pensions (disability and old age pensions) death and funeral, salary loans.

How to register as an Employee
A prospective employee who wants to register with SSS for the first time must follow the following steps;

Fill out form SSS Form E-1 (Personal Record)

Submit the original/certified true copy/photocopy of any of the following;
* NSO Birth Certificate
* Baptismal certificate
* Passport
* Driver’s License
* Professional Regulation Commission (PRC) Card
* Seaman’s Book.
In the absence of the above, you may submit two valid Identification, which one must show your picture and date of birth.

For married women, submit a certified true copy of your marriage certificate upon registration. If with children, you have to submit a photocopy of their birth or baptismal certificate.
How to register as SSS Self-Employed Member?
SSS Form RS-1(Self-Employed Data Record) must be accomplished and submit it together with a photocopy of any of the following;
NSO Birth Certificate, Baptismal, Passport, Driver’s license, Professional Regulations Commission (PRC) Card or Seaman’s Book.
In the absence of the above, you may submit two secondary identification
For Self-Employed members who have employees, you must register also as an Employer and
Secure an Employer ID number which you may use in all SSS transactions.
How to register as SSS Voluntary Member?
Separated Members
For members who are separated from their company or who ceases to be Self-Employed or as OFW, or as a non-working spouse, you may continue to pay your monthly contribution using SSS Form RS 5 using your previously assigned SSS number. Posting of your payment will automatically change your membership status from covered employee, self-employed, OFW or non-working spouse to a voluntary member.
Non-working Spouses
For non-working spouses, you have to fill out and accomplished SSS form NW-1 (Non-Working Spouse Record) and submit it duly signed by your working spouse and a certified true copy of your marriage certificate.
Overseas Filipino Workers
An OFW who wish to continue their SSS coverage must also accomplish OFW Form OW-1 (Overseas Worker Record) and submit it together with a photocopy of any of the following; NSO Birth Certificate, Baptismal certificate, Driver’s License, passport, Professional Regulation Commission (PRC) Card or Seaman’s Book. In the absence of these documents, you may present two valid secondary identification as mentioned earlier.
Some members wish that membership should be optional, without taking into consideration the numerous benefits one member can receive in their lifetime.
